MONROE TOWNSHIPOFFICE OF EMERGENCY MANAGEMENT

 


 

 

The Monroe Township Police Department, working together withMiddlesex County has developed a voluntary Special Needs Registry.  Residents who would like to register must
complete a Special Needs Registry from. 
The information provided on the form will be kept on file with the Monroe
Township Police Department and the Middlesex County Office of Emergency Management.  Should there be an emergency situation in
Monroe, this information will be accessed by the Township Police or the Office
of Emergency Management and the necessary steps will be taken to help
accommodate those individuals with disabilities who have registered by
completing and submitting the form.  Ifyou are interested in registering, please return the form to

TO PRINT SPECIAL NEEDS FORM FROM POLICE WEBSITE:

 


  1. WWW.MONROETWPPOLICE.ORG

  2. On left side of window, click “Forms andPermits”

  3. In middle of window, under Emergency Management,Township Special Needs Registry, click “Open PDF File.”

  4. If print option is not available, right clickwindow.  A mini-menu is displayed.  Left click “Print.”  In this Print window, scroll down and click“OK”.

  5. This file is a PDF file and cannot be saved toWORD unless you have a conversion program. 
    It is available only to read or print.
